Thankfully serious injury is very rare but be prepared if you get one. Top tip - print out the form and have it in your kits bag. Getting info at the game is critical.
If you have a reportable injury in your match it is essential that you complete the Serious Injury Report Form.
Reportable Injury Events
These are defined as:
• An injury which results in the player being admitted to a hospital over night (this does not include those that attend an Accident or Emergency Department and are allowed home from there so if they go by ambulance to hospital, stay in A&E for a hours and are then sent home its NOT reportable).
• Deaths which occur during or within 6 hours of a game finishing.
Clubs and schools are also required to report injuries in which artificial grass pitches or equipment such as sports goggles are involved. It does you no harm as a ref to remind people of this BUT it's not your issue to follow up on, you could put it in the injury report though.
Following a reportable injury the referee should complete Referee Injury Event Form. More information for referees following a reportable incident and details on how to complete the form below:-
The report should :
- include a short factual summary of what happened
- record details of the teams, team captains, touch judges, referee, coaches, and any medical staff including other relevant people if applicable
- not contain any speculation, opinion or guesswork
- be in the referee's own words and should reflect the language used by the referee
